Payment Instructions
Payment can be made using Zelle, Venmo or a check for the invoiced total. To Zelle or Venmo, use the email address of your provider on the Provider page. If paying with a Check please make a check payable to your anesthesiologist. Remit to the address on your invoice upon receipt.
Credit card payment requires a 3% processing surcharge, which can be calculated below:
